This form is used to recognize volunteers and enter their name into a scholarship drawing.

Volunteers may submit their own name.

Members may nominate another volunteer.

Each volunteer will receive one entry, regardless of how many times they are nominated.

Applicants who have received a retreat scholarship within the past five years are not eligible.

This event’s own time and location are listed above. Most Guild gatherings are at Minnesota Valley Unitarian Universalist Fellowship (MVUUF) in Bloomington — a few meet in members’ homes or online.
